To make a one time donation or to set-up an automatic monthly donation using a credit card:

• or print and fill out our Donation Form (CC) and mail it in to the address below
• or print and fill out our Donation Form (CC) and fax it to: 1-773-751-5110

To make a one time donation using a check:

• please print and fill out our Donation Form (Check) and mail it in with your check to the address below

To set-up an automatic monthly donation using a bank account:

• please print and fill out our Donation Form (Bank) and mail it in with a voided check or deposit slip to the address below

How to Change an Ongoing Donation

To make a change to an automatic monthly donation using a credit card:

• please fill out our Secure Online Donation Modification Form
• or print our Donation Form (CC), fill out your name, phone and information you want to change and mail it in to the address below or fax it to: 1-773-751-5110

To make a change to an automatic monthly donation using a bank account:

• please print and fill out our Donation Form (Bank) and mail it in with a voided check or deposit slip to the address below (any change requires a new authorization)
